Over twenty (FDA-approved) drugs have been recalled for safety reasons over the past decade.

The Federal Centers for Disease Control and Prevention reports that. adverse reactions "poisoning" from prescription drugs has risen to become the second-largest cause of unintentional deaths in the United States,

An example of the results of recalls includes  things like the FDA's senior drug safety researcher, Dr. David Graham, (whistleblower) on the drug Vioxx (just one example) caused over 140,000 heart attacks in patients, likely killing more than 60,000 Americans who took the drug.

Yes this drug was fully approved by the FDA and court damage cases results are making it clear why, and the slight of hand used in getting it approved.

More than 600 drug-safety alerts (on previously FDA approved drugs) and over 250 black-box warnings were issued during that time.* 

From Wikipedia, A black box warning means that medical studies indicate that the drug carries a significant risk of serious or even life-threatening adverse effects. The FDA can require a pharmaceutical company to place a black box warning on the labeling of a prescriptiondrug, or in literature describing it.

Note: The FDA allows continued sales, just that you must now add a black bordered label enclosed with the drug so you are "warned" of the dangers.  

Recent examples, Diabetes drug Avandia over causing increased risks of heart failure.  Celebrex is another one you may have heard of. Black box warnings ordered placed on all antidepressant medications warning they may result in increased risk of suicidal tendencies in children and adolescents.

The FDA often approves drugs before safety has been established and then requires the drug company to conduct post-marketing studies. Drug companies are notoriously slow about ever doing so.

Generations Magazine on the subject of pharmaceuticals and the elderly reports that medication-related problems may be the third or fourth leading cause of death in the over-65 age group. Other articles in the same issue discuss these issues.

Abuse of prescription medications among the elderly. for example, elderly persons who take benzodiazepines are unknowingly being put at increased risk for falls that cause hip and thigh fractures, as well as for vehicle accidents and cognitive impairment.
